We receive many emails asking specific questions about HG. In order for us to best answer your questions, it is helpful to know some general information about your present situation and past medical history.
All but the last few questions are optional. Answer to the extent you are comfortable, so we can respond more efficiently. One of our health professionals usually responds within 1 business day to the questionnaires, depending on the volume of email we receive.
This information is only seen by our health professionals. It is not collected in a database or used in any other manner, and it is never shared or discussed without your express permission. You may receive an occasional news update on HG which you can opt out of at any time.
